During the SOCA BP 2018 Renewal process, Brokers will need to send Anthem the signed renewal rates for the group through FormFire for all groups making changes. Employees are responsible for updating their FormFire accounts with changes during the renewal period. Once complete, brokers will follow the steps below to send the changes and a copy of the signed renewal documentation directly to Anthem through FormFire.
Note: Prior to sending forms, please make sure all 2018 plans and pricing have been updated in FormFire via Assign Plans, and all employee plan selections have been made via group enroll. This MUST be done even if the group is only offering one plan. For instructions on assign plans and group enroll, please refer to these help articles:
From your Client Dashboard, locate the client and click on the orange box, then select Send Forms.
Note: All employees must be Complete to allow you to proceed to this step. If any employees are showing as Updating or Not Started, they will need to login and complete their form before you can proceed further.
Select Sold Group as the submission type.
Select Anthem for SOCA BP Renewal as the Carrier.
Select Anthem for SOCA BP EE App (Renewal) as the form.
Scroll down to the Attach Files section. Select Upload Attachment to upload a copy of the signed rates / benefits for the 2017 SOCA BP Renewal to send with your submission.
Select Benefit/Rate Proposal as the file type. Make sure you have the signed rates for the SOCA BP Renewal saved to your desktop. Select Choose File to locate the file and attach it to your submission. Finally, select Upload File to attach the document to your submission.
Once attached, it will appear in Attach Files section. Make sure to uncheck any files you do not want to send with your submission at this time. The only files that must be checked are the signed renewal rates and the automatically generated renewal census, both shown checked below.
Once all required documentation is attached, scroll down to the bottom and select Send Forms to send the files to Anthem. FormFire has prefilled the correct information for renewal submissions in the recipient fields to make sure your forms are sent to the correct location for the SOCA BP Renewal.
