Who can see my information?
Your information will only be viewed by your company's designated benefits broker and insurance carriers providing a health insurance price quote.
Your employer cannot see your information.
How long does my personal information remain in your system?
Personal information will be in FormFire in some form indefinitely depending upon the level the of user's activity. After two years of no activity, we reserve the right to permanently delete the data without the user's permission. However, we are required to store copies of the digitally populated forms for seven years.
What is your policy regarding the removal of inactive accounts?
After two years of inactivity, we reserve the right to permanently delete the account without the user's permission.
Can I request that my account be deleted?
Yes. We will permanently delete a user's account upon receipt of a request with the following information:
- Your name as it appears on your account
- Your FormFire user ID
- The last five (5) digits of your social security number
- Date of birth
You may email the request to firstname.lastname@example.org with Account Deletion Request as the subject line.

Please Note that deletion requests are permanent and cannot be undone.
When an account is deleted, does it remain in the backup files?
Our IT strategy dictates that information will be kept for one month in backup media after an account is deleted.
Will the user be notified each time any information is provided to a third party?
