1095 Tax Forms
In the Southern Ohio Chamber Alliance Benefit Plan, every employer participating in the plan is responsible for filing the 1094 and 1095 forms with the IRS to satisfy their requirements as a plan sponsor of a self-funded health benefits plan. The employer is responsible for completing the 1094 and 1095 forms, providing documents to its employees, and submitting documents to the IRS. Please contact your tax, legal or payroll representative for further assistance.
The Plan Administrator has provided a 1095 tax form, available for download in FormFire from the HR manager's Document Center. FormFire, LLC does not make any representations as to the completeness or accuracy of the information provided.
How to download the 1095 Tax Form
Find Documents on the HR Dashboard and select Manage Documents.
You'll find the 1095 Tax Form in your list of documents. Because the report contains sensitive information, it is password protected with the employer's 8-digit Anthem group number (e.g., 00123456). Download the form and enter the password to access the information. If you do not remember your 8-digit Anthem group number, please contact your Anthem representative.
