Resetting your password has never been easier! Please follow the steps outlined below to reset your password online. Begin by clicking here.
You will have 2 options to reset your password:
- Personal Information
You'll be directed first to use your Email Address.
Email Password Reset
Here, enter in the Email Address that is associated with your FormFire account.
Once entered, you will see a screen that looks like this:
Be sure to stay on this screen until you receive your code! If you navigate away, you will not be able to return and will need to start the process, again. Type in your code and you will be prompted to create a new password.
When you choose to reset your password through your email address, FormFire will send you an email with your username along with the 6 digit security code.
Personal Information Reset
Enter in your Last Name, Date of Birth and last 4 digits of your Social Security Number (SSN) in order to reset your password.
NOTE: Please make sure to format your date of birth as mm/dd/yyyy. Only entering in the last 2 digits of the year will not allow the reset to occur.
Whichever option you choose to reset your password, you will be logged out of the account once you have successfully reset your password. We'll prompt you to log back in with your new password!
Why can't I reset my password?
Passwords will not reset unless the information you have entered is exactly the same as what is in your FormFire account. For instance, if you have switched employers, you may find that your email has changed as well as the employer code.
