Follow the steps below for all employee terminations for any participating employer in the Anthem for SOCA Benefit Plan.
First, the HR Manager must log into FormFire and click Manage Employees on the HR Dashboard.
Next, find the terminated employee in the list, select the employee and click Edit.
This will take you to the Employee Status page. Scroll down to the Status section and change the employee's Employment Status from Active to Pending Continuation. This must be done for all employee terminations. This change will yield a series of additional questions used so MyCOBRA Plan can send the terminating employee the correct continuation paperwork.
Answer the following Questions:
- Has the employee been terminated?
- Termination Date?
- Termination Type (i.e. Voluntary, Involuntary, or Death)?
- Was the employee enrolled in benefits for 3 months prior to their termination?
Once all these questions have been answered, select Save.
Clicking Save should return you to the Manage Employees page where the terminated employee should show as Pending Continuation instead of Active (shown circled below).
Both the HR Manager and the Broker can receive the Termination Confirmation email from email@example.com. If you do not receive this email, you can turn on this notification under Account Settings (for HR Managers) or Account Setup (for Brokers). The Notification email will contain an attached termination form which must be sent to Anthem to notify them of the termination. Medical Termination Notifications should be sent to Anthem for processing via fax at 1-800-883-7919. Alternatively, applications can be submitted via Anthem's E-Submit process.
If the terminated employee later elects continuation coverage, repeat the previous steps, however, change Pending Continuation to either State Continuation or COBRA, whichever is applicable, based on your employer's qualifications. Depending on which choice is selected, there may be different questions triggered. Answer all questions applicable for that employee.
