When sending prescreen and sold group submissions to the Anthem for SOCA BP, the provided checklists will help identify required documents that have been or need to be added to your submissions.
After selecting Anthem for SOCA BP as the carrier on the Send Forms page, you'll see that the Rules of Engagement and the prescreen Checklist will appear on the right-hand side of the screen.
Only 3 items are required for the prescreen submission to the Anthem for SOCA BP:
- SOCA BP employee applicatins
- Employer Questionnaire
- Census of all eligible employee
FormFire will auto-load all 3 required documents for the prescreen submission.
NOTE: Brokers are responsible for completing the Employer Questionnaire for their clients in FormFire.
If you answered Yes to the question Is your company currently enrolled in a self-funded (ASO) or level-funded arrangement? on the Employer Questionnaire for your client, you'll see an additional item appear on the prescreen Checklist.
If your client is in a self-funded (ASO) or level-funded arrangement, make sure to either load, or request, the required additional documents in your client's Document Center.
NOTE: Brokers won’t be able to send to Anthem until all ASO items have been added under the appropriate category!
Sold Group Checklist
After selecting Anthem for SOCA BP as the carrier on the Send Forms page, you'll see that the sold group Checklist will appear on the right-hand side of the screen.
FormFire will auto-load 3 documents for the sold group submission:
- SOCA BP Employee applications
- SOCA BP Employer application
- Anthem enrollment census
Brokers are responsible to load 6 documents:
- Participation Agreement
- Benefit Rate Proposal
- Wage and Tax Statement
- Association Membership/Proof
- Electronic Funds Transfer (EFT) banking form
- Voided check
All items can be loaded, or requested, in your client's Document Center.
This new checklist will now indicate whether you've attached all required documents in the Attach Files section:
- A document with a green check markindicates an item that has been loaded
- A document with a yellow exclamation point indicates an item that hasn’t been loaded
NOTE: Brokers won’t be able to send to Anthem until all items have been added under the appropriate category!
