When sending Prescreen and Sold Group submissions to the Anthem for SOCA Benefit Plan, the provided checklists will help identify required documents that have been or need to be added to your submissions.
Prescreen Checklist
After selecting Anthem for SOCA Benefit Plan as the Carrier on the Send Forms page, you'll see that the Rules of Engagement and the Prescreen Checklist will appear on the right-hand side of the screen.
Only 3 items are required for the Prescreen submission to the Anthem for SOCA Benefit Plan:
- SOCA Benefit Plan Employee applications
- Employer Questionnaire
- Census of all eligible Employees
FormFire will auto-load all 3 required documents for the Prescreen submission.
If you answered Yes to the question Is your company currently enrolled in a self-funded (ASO) or level-funded arrangement? on the Employer Questionnaire for your Client, you'll see an additional item appear on the Prescreen Checklist.
If your Client is in a self-funded (ASO) or level-funded arrangement, make sure to either load, or request, the required additional documents in your Client's Document Center.
NOTE: Brokers won’t be able to send to Anthem until all ASO items have been added under the appropriate category!
Sold Group Checklist
After selecting Anthem for SOCA Benefit Plan as the Carrier on the Send Forms page, you'll see that the Sold Group Checklist will appear on the right-hand side of the screen. This new checklist will now indicate whether you've attached all required documents in the Attach Files section as well as ensure that the all Employees selected their plan(s) and that you as the Broker added pricing information.
Brokers must ensure that the following steps are complete:
- Employee Plan Selection
Brokers are responsible to load 7 documents:
- Participation Agreement
- Benefit Rate Proposal
- Association Membership/Proof
- Electronic Funds Transfer (EFT) banking form
- Voided check
- Business Acknowledgement Agreement
- Annual Rate and Census Reconciliation (ARC) Form
FormFire will auto-load 4 documents for the Sold Group submission:
- SOCA Benefit Plan Employee applications
- SOCA Benefit Plan Employer application
- Anthem Enrollment Census
- Ancillary Coverage Line Documents
You'll see 2 icons in this checklist:
- A green check mark
indicates an item/requirement has been loaded/completed
- A yellow exclamation point
indicates an item/requirement hasn’t been loaded/completed
