How can I as the Broker help my client complete the Employer Questionnaire?
Brokers can now complete the Employer Questionnaire on the Broker Portal! The Employer Questionnaire will be split into 3 sections:
- Employer Information
- Prior Medical Coverage
- Eligibility, Participation and Contribution
NOTE: To submit employee data for purposes of completing a Prescreen with Anthem SOCA, you must complete ALL information on this screen before sending forms to Anthem.
For the Employer Questionnaire, what happens if the client is not currently a chamber member, but will become a chamber member if the premium makes sense?
Brokers can select the chamber the client plans to become a member of, however will need to submit proof of chamber membership with the Sold Group submission.
The HR Manager cannot complete the Employer Questionnaire?
Correct, the Employer Questionnaire must be completed by the Broker. But you can review the questions listed on the Employer Questionnaire with your client to make sure that you're answering them correctly.

I'm an HR Manager. My Broker said that I need to fill out an Employer Application. Where is this?
You can access the Employer Application right from the HR Dashboard. To access the Employer Application, click the brown Anthem SOCA ER Application button under Employer Medical Info. The Employer Application will be split into 6 sections:
- Health Plan
- Employer Information
- Contribution Requirements
- Review Terms and Conditions
NOTE: To submit employee data for purposes of completing a Sold Group with Anthem SOCA, you must complete ALL information on this screen before sending forms to Anthem. However, if you need to leave this screen before completing, please click Save in order to save your progress.
Can a Broker complete the Employer Application or just monitor the status?
Brokers can fill out the Employer Application. However, they would have to sign in as the HR Manager with their permission to do so. The HR Manager should still sign the Employer Application in the Signature section to complete.
Does system allow selection of two plans for dual option?
Yes! The HR Manager will select the plans in the dual option in the Health Plan section of the Employer Application.
Does the Employer Application still require Broker signature?
Yes, a Broker's signature is required on the Employer Application. FormFire will map the signature of the Broker listed as Team Member 1 on the Client Details page for the client.

Prescreen & Sold Case Checklists
Why can't I send my Sold Group submission to Anthem?
There are three scenarios that will prevent you from sending a Sold Group Submission to Anthem.
Scenario 1) You haven't entered in pricing information for the medical plan(s) or the employees haven't selected plans.
Scenario 2) You haven't added one of the 7 required Broker Uploaded Documents. Brokers are required to add the following items in their client's Document Center for the Sold Group submission:
Scenario 3) The HR Manager hasn't completed the Employer Application
Refer to the Sold Case Checklist on the Send Forms page for clarification:
- A green check markindicates an item/requirement has been loaded/completed
- A yellow exclamation point indicates an item/requirement hasn’t been loaded/completed
Is the Business Acknowledgment Agreement still needed for submission?
The Business Acknowledgment Agreement is still required and still needs to be emailed to firstname.lastname@example.org and a copy must be saved in the Document Center.
Is there an "Other Documents" section for the sold checklist?
You must upload the required documents using the correct document type from the drop-down when you upload/attach documents. If you load a required document as "other", the system will not recognize that is has been loaded as one of the required documents.

How can I terminate an employee from my client?
Now, Brokers and HR Managers have the ability to terminate an employee from a client in FormFire.
- Brokers can terminate an employee from their client's Employee List page
- HR Managers can terminate an employee from their Manage Employee page
NOTE: This process is to terminate the employee's coverage, not to terminate their employment at a company.
Do both the HR Manager and the Broker enter the termination information for the same employee?
An employee only needs to be terminated ONCE in order to begin the Termination Process. Brokers and HR Managers should discuss amongst themselves as to who will be terminating the employee.
Does the termination automatically get sent to Anthem or do I have to manually send the termination to Anthem?
Completing the Termination Process through the FormFire site only updates myCobraPlan. You're still required to send the termination paperwork to Anthem as usual.
I see that "pending continuation" is no longer an option for an employment status. The next time HR Managers login to terminate an employee, is there some sort of callout as to these changes in the system?
Currently, there are only 2 employment statuses that an employee can have- Active or Terminated. The HR Guide for the Anthem for SOCA BP and our Help Articles on the Termination Process will highlight the new employment statuses.
Does my client have to use Anthem's COBRA administrator?
myCobraPlan is the exclusive continuation provider for the Anthem for SOCA BP. Participating employers are required to use myCobraPlan.
If an employee is terminated and removed from my client, where do they go? And what happens if the employee is rehired back to the same employer?
The employee's account is moved to the individual's list for your agency. If the employee is hired back, you can move the account back to the employer's account.
If an employee voluntarily leaves employment, should I select 'yes' to question regarding COBRA / State Continuation?
If the employee voluntarily leaves their employment, select Voluntary from the Termination Type drop-down menu and enter the termination date. You should keep the answer as Yes to the question regarding COBRA / State Continuation.
If an employee voluntarily leaves employment, are they eligible for State Continuation?
The employees’ information will still need to be entered. However, the employee won't receive an election notice if they are not eligible for continuation coverage.
If an employee is removed, do they still receive the COBRA / State Continuation information?
Yes, the employee will still receive COBRA / State Continuation information. This is due to the fact that the employee must be terminated prior to being removed from the client.
If employee terminates participation in the plan, but they are still employed, do you still show employee's employment status as terminated, but then not remove them from the client?
If the employee is terminated from the plan due to reduction in hours, you would show them as terminated, but keep them in your client. However, if the employee is coming off the plan to enroll in their spouse's coverage, they will need to update their FormFire account and the Broker/HR Manager will need to fax the Anthem SOCA Change Form directly to Anthem.
What happens if you only want to terminate a dependent?
The employee should log in to the interview and remove the dependent as a QLE.
Why would you want to remove/delete a terminated employee from the client? If removed, what happens if that employee elects to continue coverage?
We have several employers that remove employees after their coverage has been terminated. It is up to the employer's discretion. Because the employee must be terminated prior to being removed, the employee will still receive COBRA / State Continuation information.
Why would I not want to remove a terminated employee from the client?
If the employee elected State Continuation or COBRA, you'll want to keep them in the client for tracking purposes. We have several employers who remove terminated employees from their group after their coverage has been terminated. It is up to the employer's discretion.
