If you give consent to one of the following individuals, they can assist you in completing your account online:
- A spouse
- A domestic partner
- A child
- A parent
- A family friend
- Your company’s HR Manager
- Your company’s Insurance Broker
Anyone you give consent to is considered a third party.
If the third party (HR Manager, Insurance Broker, spouse, etc) needs technical support, you’ll need to be on the phone with them when they call the FormFire Client Services Team, so your identity can be verified. You must state that you’ve given this person permission to access your account. If you are unable to call with them on the line, you can call the FormFire Client Services Team in advance to give us permission to speak with them regarding your account when they call in.
NOTE: The FormFire Client Services Team can assist a third party with any technical questions they have for only 24 hours after you’ve confirmed they have access to your account.
The FormFire Client Services Team cannot complete your account for you, but they’ll assist with any technical questions that you have.
