Brokers who are submitting Prescreen and Sold Groups submissions to the Anthem for SOCA Benefit Plan should follow the steps outlined in this article to successfully set up their Broker accounts in FormFire.

Creating and signing your account
All Brokers that use FormFire should have their own accounts within the site. Specific documents for the Anthem for SOCA Benefit Plan require the agent of record's signature and Anthem Broker ID. FormFire will be able to map this information based the Broker's account that is listed as Team Member 1 for a specific Client.

FormFire requires that Brokers complete the following areas of the Employee Interview:
- FormFire Profile: You're not required to fill out any dependent information
- Coverage: Select Waive for all coverage lines
- Sign & Submit: This is where you’ll sign your account

Adding your Broker ID
The Anthem for SOCA Benefit Plan requires Brokers to enter their Broker IDs on specific documents. FormFire will map this information onto the required documents for you, once it has been added into the system.
