FormFire is currently Closed means that your company has already completed either shopping for benefits or its open enrollment period.
If you're accessing your account during this time, you're usual do so for one of three reasons:
- You're a new hire at your company and need to complete your insurance form
- You need to make a Qualifying Life Event change to your existing account
- You need to make a minor or non-Qualifying Life Event change to your existing account
You're a new hire
If you’re new to your company, click the orange Enroll Now button and you’ll be able to enter in your information. For more information on how to navigate your account, click here.
You need to make a Qualifying Life Event Change
A Qualifying Life Event (or QLE) is a change in your situation — like getting married, having a baby, or losing health coverage — that can affect your insurance.
If you need to make a benefit coverage change for yourself or a dependent, select a reason from the Reason for making a change drop-down menu:
- Had a baby
- Adopted a child
- Granted custody / Guardianship
- Got married
- Got divorced
- Dependent death
- Loss / Gain of coverage
NOTE: Selecting a reason will require you to either add or delete a dependent. This depends on the type of reason you select.
Once selected, click the orange Request Benefit Change button to enter your account. Then, you’ll be able to update your information. You can follow the steps outlined in How to navigate the Employee Portal.
You need to make a non-Qualifying Life Event Change
If you need to make a non-coverage change, such as updating your address or basic personal information, click the orange Update Info button under Non-Coverage Changes. This will allow you access to your account to make the necessary changes.
NOTE: This option WON’T allow you to change your benefits, medical history or that of your dependents.
Then, you’ll be able to update your information. You can follow the steps outlined in How to navigate the Employee Portal.
NOTE: Regardless of the reason you need to access your account outside of your company’s enrollment period, you MUST sign for your changes to be complete.
