Generate the Census
First, you'll log into your FormFire account using your username and password. Once in your account, you’ll see the Client Dashboard. Find the Client you’d like to quote Beam Dental for, click the orange menu box and select Generate Census.
Here, you’ll see two drop-down menus.
- In the first, select Quote Census.
- In the second, select Beam Dental Quote Census.
Then, click Download Census. An excel census file will be downloaded on your computer.
Now that the census is generated, click the Plan Rater tab to review. The following items should be filled in automatically:
- Client Zip
- Group State
- SIC Code
- EE (Employee) Count
The EE (Employee) Count comes from the total number of Employees in FormFire for the Client. If this section is not displaying the correct number, please check the Employee List page for this Client to see if all Employees have created their accounts online.
All available plans are listed on the Plan Rater tab. A cover page and additional SBC documents are available on the other tabs that you can add to the final proposal that you’ll create for your Client. To create the proposal, you can either print the specific tabs you want to include or save the specific tabs as a PDF. Make sure that you save the downloaded quote file to your computer. You'll need it for the enrollment!
Assign the plan through Group Plan Assign
After you’ve determined which Employees from the Client would like to enroll into the selected plan, it’s time to assign them. Starting from the Client Dashboard find the Client once more from your list. This time when you click the orange menu box, select Group Plan Assign.
When here, click on the Dental tab. You’ll add the plan needed in this section.
- Select Beam Dental as the Carrier
- Select the plan you need from the plan list
Then, click the box by Last Name and the plan will be assigned to all Employees that elected Dental coverage.
Once the plan has been added, you'll click the Complete Plan Assignment button. Read the Confirmation notice that appears on the screen and click Complete.
Send the submission to Beam Dental
After you've assigned the plan, you can submit the Client to Beam Dental. Find the Client once last time from your list. This time when you click the orange menu box, select Send Forms.
Here, select the following:
- Submission Type: Sold Group
- Carrier: Beam Dental
- Form: Select the form that pertains to the state the Client is located
Leave Dental as the only boxed checked under Coverage lines to include.
Add in the Notes to Carrier Plan selected = name of the plan the Client went with.
Add the previously downloaded quote file you create and add it in the Attach Files section.
Lastly, click the orange Send Forms button. Review the items on the confirmation pop-up and click the blue Send Forms button.
After the initial enrollment, any Qualifying Life Events and enrollment changes for your Client, head over to Lighthouse, Beam’s benefits/ongoing management tool at Lighthouse.beam.dental.
