Who can see my information?
Your information will only be viewed by your company's designated benefits Broker and Insurance Carriers providing a health insurance price quote.
Your employer cannot see your information.
How long does my personal information remain in the FormFire system?
Personal information will be in FormFire in some form indefinitely depending upon the level the of user's activity. After two years of inactivity, we reserve the right to permanently delete the data without the user's permission.
However, digitally populated forms that have been sent to insurance carriers for the purpose of quoting and enrolling in insurance benefits will be securely maintained and encrypted. All forms and data that an insurance carrier retrieves from FormFire will remain subjected to HIPAA regulations.
What is your policy regarding the removal of inactive accounts?
After two years of inactivity, we reserve the right to permanently delete the account and the data entered into said account without the user's permission.
Can I request that my account be deleted?
Yes. You may email the request to support@formfire.com with Account Deletion Request as the subject line. We will permanently delete a user's account upon receipt of a request with the following information:
- Your name as it appears on your account
- Your FormFire username
- The last four (4) digits of your social security number
- Date of birth (MM/DD/YYYY)
When an account is deleted, does it remain in the backup files?
Our IT strategy dictates that information will be kept for one month in backup media after an account is deleted.
Will the user be notified each time any information is provided to a third party?
FormFire does not sell or make user information available to any third parties. For a more detailed information about privacy, please read our Privacy Statement.
