How to send a Prescreen submission to Anthem MCF BP

Once you've collected the medical information from the Employees and the Employer Questionnaire is complete, you're ready to submit your Client's applications via FormFire to Anthem Missouri to obtain a Prescreen medically-underwritten quote.

Initial Prescreen Checklist

• MCF BP Employee Applications
• Census of All Eligible Employees
• MCF BP Employer Questionnaire
• If ASO: Claims, Large Claims, and Fixed Costs (This will need to be added to the Document Center)

Submitting forms through FormFire

Follow the steps below to properly submit your forms to ensure it will be processed more efficiently and to expedite the process.

From your Client Dashboard, locate your Client and on the green context menu, select Send Forms.

Now, you can set up the Prescreen submission.

• Submission Type: Prescreen
• Carrier: Anthem MCF BP
• Form: MCF BP EE Employee Application

In the Notes for Carrier section, you MUST type the name of your Client's chamber association here when sending your submission to Anthem to obtain a prescreen quote for the MCF BP.

Select any Additional Attachments from the Attach Files section that you'd like to include with your submission. These items will also be stored in the Document Center.

You won’t have to worry about entering in the Recipient information – this is going directly to Anthem MCF!
