It's fairly easy to turn on the Employer Questionnaire & Application for your Client. To do this, log into your account and locate your Client from your Client Dashboard. Open the orange context menu and select Client Details.
Scroll down to the Employer Options and make sure Enable Employer Group Medical Questionnaire has the check mark next to it.
Make sure that you click the blue Save button at the top of your screen for your Client to be saved in FormFire.
