A family member includes a:
- Domestic partner
- or someone who may be covered under your benefits
A family member doesn’t include a(n):
- or someone not covered under your benefits
NOTE: If a family member is waiving (not taking) all coverages that your employer is offering you, you don't need to add that person's information into FormFire. You only need to add family members who'll be taking some or all coverages with you.
You’ll enter your family members' coverage and plan selections as well as their medical history. Just as with your information, you’ll only enter in once and then update as needed every year you use FormFire.
