How to set your Broker account for the SummaCare Chamber HBP

Brokers who are submitting Prescreen and Sold Groups submissions to the SummaCare Chamber HBP should follow the steps outlined in this article to successfully set up their Broker accounts in FormFire:

• Creating and signing your account in FormFire
• Add their National Producer ID

Creating and signing your account

All Brokers that use FormFire should have their own accounts within the site. Specific documents for the SummaCare Chamber HBP require the Agent of record's signature and SummaCare National Producer ID. FormFire will be able to map this information based the Broker's account that is listed as Team Member 1 for a specific Client.

FormFire requires that Brokers complete the following areas of the FormFire Interview:

• FormFire Profile: You're not required to fill out any dependent information
• Coverage: Select Waive for all coverage lines
• Sign & Submit: This is where you’ll sign your account

Adding your National Producer ID

SummaCare Chamber HBP requires Brokers to enter their National Producer IDs on specific documents. FormFire will map this information onto the required documents for you, once it has been added into the system.
