FormFire has an impressive catalog of available Carrier plans in our Plan Library. But what happens if you need a specific plan that's not currently listed?
It's fairly simple! You’ll need to complete our online FormFire Carrier Plan Request form.
This online form will ask you to provide a clean copy of the Carrier’s plan summary (SOB or SBC) in .pdf formatting as well as the following information below:
- The state or states that this plan will be available in OR the name of the Client as listed in FormFire
We value our Brokers and their Clients confidentiality, so please do not include:
- The Client's renewal
- The Carrier's proposal
- Any emails between you as the Broker and your contact at the Client

FormFire's standard turnaround time for a one off plan request is up to 5 business days, if all information is sent over in the initial request.
The FormFire Plans team will provide you with direct updates regarding your request.
NOTE: Plan turnaround times are subject to change. The FormFire Plans team will notify you with their current turnaround time or if there issues that prevent your request from being completed.
